Screening, Assessment, and Diagnosis

At Eastridge Health Systems, we understand that taking the first step toward seeking help can feel overwhelming, which is why our screening, assessment, and diagnostic services are designed to be supportive, thorough, and personalized. These services are often the first stage in a person’s care journey and play a vital role in helping our clinicians understand individual needs.

During the screening process, our team gathers important background information and listens to the concerns that led you to seek support. This helps us identify the timing and nature of the more comprehensive assessment that is needed to begin services with us. During this comprehensive assessment, our clinicians use a combination of evidence-based tools, structured interviews, and professional expertise to evaluate mental health conditions, substance use concerns, and overall functioning in order to develop treatment goals that meet your needs.

A key part of this process includes conducting risk assessments, which allow us to evaluate any immediate safety concerns, such as risk of harm to self or others. By addressing these risks early, we can connect individuals with the right level of care and ensure that urgent needs are met quickly and appropriately.

The outcome of these services is a clear, individualized understanding of each person’s strengths, challenges, and goals. With this information, we work together to develop a personalized treatment plan that may include counseling, psychiatric care, community support, and/or referrals to other services. Our goal is not only to identify what someone is struggling with, but also to provide a roadmap toward stability, recovery, and improved quality of life.
